Just trying to get a feel from others about the typical level of effort for
getting a theme right (I know that's a subjective measure).

In your experience how long does theme creation take in the following
scenarios:

1. Starting from just a design in a design program
2. Starting from valid XHTML/HTML (e.g. the stuff from OSWD.org)
